Medical Services: All clients are eligible for medical evaluation. Financial assistance is available for laboratory, medicine, and doctor, RN, nurse practitioner visits per guidelines.
Case Management: All clients are assessed and care plans are established.
Nutritional Evaluation and Counseling.
Mental Health and Substance Abuse Referrals.
Limited Dental Services.
